354 Surgical Proforma Done by Surgical Doctors; Clinical Audit

Abstract Aim To improve the clerking performed by all surgical grades at our Trust and to prevent any implication might happen in case was insufficient such as medical errors, delay treatment poor quality of data for future research audits. Method Two cycle audit which first analysed one hundred fully randomized entries completeness aspects history, examination & plan over two months 2021. Then new proforma applied used patients thereafter. Subsequently, second later, same randomization applied. Results Whereas completed proformas compromised only 4% cycle, this percentage increased more than six-fold reach 26% one. Both history present illness physical exam showed major improvement. Even though some sections NEWS score social kept showing small, percentage, it slightly higher cycle. 69% total improvement across between cycles. Conclusion New hospital improved done doctors hence care provided place served a very rich environment Audits.
